Your CSV must include these columns:
- First name
- Last name
- Professional Registration Body
- Professional Registration Number
- Health Board
- Prescribing Start Date
- Prescribing End Date
Required fields: First name, Last name, Professional Registration Body, Professional Registration Number, Health Board, and Prescribing Start Date.
Health Board must be set to All.
Enter all dates in the format dd/mm/yyyy (for example, 25/12/2026).
Maximum number of rows is 1,000.
